Trampolines Recalled by Skywalker Holdings; Enclosure Problems Can Result in Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: 13-foot Square Trampolines

Units: About 60,000

Distributor: Skywalker Holdings LLC, of Brigham City, Utah

Hazard: The straps supporting the top of the trampoline’s enclosure to the poles can fail. The enclosure could drop if these straps break, posing a risk that a user could fall from the trampoline.

Incidents/Injuries: Skywalker Holdings has received at least 250 reports of straps breaking. No injuries have been reported.

Description: This recall involves the Skywalker Holdings 13-foot Square Trampoline and Enclosure Combo. The unit has blue spring pads, a black net enclosure, and a jumping mat. “Skywalker Holdings” is printed on a label located under the jumping mat and on the enclosure net.

Sold at: Specialty stores and major retailers nationwide and online from January 2007 through February 2009 for between $400 and $600.

Manufactured in: China

Remedy: Consumers should immediately stop using the trampolines and contact Skywalker Holdings to obtain a free repair kit.

Ross Stores Recalls Benches Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: 39” St. Anne Acacia Benches

Units: About 225

Importer: Ross Stores Inc., of Pleasanton, Calif.

Hazard: The bench frame can collapse when weight is placed on it, posing a fall hazard to consumers.

Incidents/Injuries: Ross Stores has received two reports of incidents but no reports of injuries.

Description: The benches are made of acacia wood with SKU 400037765413 and manufacturer style number GB1F printed on a hangtag attached to the bench. The acacia wood benches measure 39” wide, 20” deep and 36” high.

Sold at: Ross Stores nationwide in March 2009 for about $90.

Manufactured in: Vietnam

Remedy: Consumers should stop using these benches immediately and return them to any Ross Store for a full refund.

Composite Decks Recalled by Louisiana-Pacific; Decks Can Deteriorate and Break, Posing Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Composite Decks

Units: About 48 million linear feet (decks vary in size)

Manufacturer: Louisiana-Pacific (LP) Corp., of Nashville, Tenn.

Hazard: The recalled decking can prematurely deteriorate and unexpectedly break. Consumers can fall through broken decking and suffer serious injuries.

Incidents/Injuries: LP has received 37 reports of composite decks breaking, resulting in 14 injuries, including a broken wrist, sprained ankle, minor lacerations and bruises.

Description: The recall includes outdoor deck board and railings sold under the brand names LP WeatherBest®, ABTCo., and Veranda®. They are composite products that look similar to natural wood and were sold in various colors including Tuscan Walnut/Chestnut, Driftwood Grey/Greystone, Pacific Cedar and Western Redwood. Veranda decking products were manufactured by multiple firms; only products manufactured by LP are included in this recall.

Sold at: The Home Depot (Veranda® brand) and building product dealers (LP WeatherBest® and ABTCo. brands) nationwide from January 2005 to August 2008 for between $1.50 and $2.25 per linear foot.

Manufactured in: United States

Remedy: Consumers with the recalled decking should immediately contact LP for a free inspection. If the decking is affected by premature deterioration, LP will arrange for a free replacement.

Dahon and REI Novara Folding Bicycles Recalled Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: 2008 model year Dahon and REI Novara Folding Bicycles

Units: About 11,500

Importers: Dahon California Inc., of Duarte, Calif. and Recreational Equipment Inc. (REI), of Kent, Wash.

Hazard: The hinge on the handlepost can crack, posing a fall hazard.

Incidents/Injuries: There have been 25 reports of the handlepost hinge on these bicycles cracking, including one minor injury involving scrapes and bruises.

Description: This recall involves model year 2008 Dahon and REI Novara folding bicycles. The models included in the recall are as follows:

Dahon Ciao P8, Curve D3, Curve SL, Glide P8, Jetstream P8, Mu P8,
Mu P24, MU SL, Mu XL Sport, Vitesse D7 HG, Speed D7,
Speed P8, Speed TR, Speed Pro TT, Mariner D7
REI Novara Buzz FlyBy and FlyBy Shuttle

The words “Dahon” or “Novara” and the model name are printed on the bicycle. Except for the Jetstream P8 and Glide P8 models, hinges with an “R” embossed on the face of the upper handlepost hinge and/or a “Radius Handlepost” sticker on the handlepost are not included in the recall. All Jetstream P8 and Glide P8 models are included in the recall unless they have an “R” embossed twice on the face of the of the lower handlepost hinge (Image 2) indicating they have been repaired.

Sold by: Authorized Dahon California dealers and REI stores nationwide from March 2008 through April 2009 for between $430 and $1,300.

Manufactured in: Taiwan and China

Remedy: Consumers should immediately stop riding these bicycles and contact any Dahon California dealer or REI store for a free inspection and repair.

Dorel Juvenile Group Recalls Safety 1st Stair Gates Due to Fall Hazard

Name of Product: Safety 1st SmartLight Stair Gates

Hazard: The hinges that hold the stair gate in place can break, posing a fall hazard to children if the gate is placed at the top of the stairs.

Incidents/Injuries: None reported.

Description: The recall involves the Safety 1st SmartLight Stair Gate with model number 42111, which is printed on a sticker under the handle panel. The gates, which were manufactured before December 31, 2007, are white metal with a gray handle and a motion sensor nightlight that illuminates on approach. The manufacture date is located below the gate’s handle as a date wheel imprinted in the plastic; an arrow indicates the month, and the year is stamped inside the circle. The manufacture date is also printed on a sticker inside the battery door.

Sold at: Mass merchandise and juvenile product stores nationwide, including Babies R Us and Wal-Mart, from January 2005 through December 2007 for about $60.

Remedy: Consumers should stop using the recalled gate and contact the company to obtain a free repair kit.

Saris Cycling Group Recalls Stationary Bicycle Trainers Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Stationary Bicycle Trainers

Units: 2,000

Manufacturer: Saris Cycling Group, of Madison, Wis.

Hazard: A handle pin on the bicycle trainer can loosen during use, causing the machine to become disengaged, and pose a fall hazard to consumers.

Incidents/Injuries: No injuries have been reported.

Description: The recalled trainers are a stationary frame that allows bicycles to be converted into stationary bicycles. The recalled models include: CycleOps Pro Series Trainer models 9014 (SuperMagneto Pro), 9331 (JetFluid Pro), 9460 (PowerBeam Pro), 9321 (JetFluid Pro Winter Training Kit), and 9322 (SuperMagneto Pro Winter Training Kit) without the secondary locking pin. The model number and style names are printed on the left side of the main frame of the trainer.

Sold at: Independent bicycle retailers nationwide from October 2008 through November 2008 for between about $400 and $1200.

Manufactured in: United States

Remedy: Consumers with the recalled bicycle trainers should immediately stop using the product and contact Saris Cycling Group to receive a replacement trainer.

Fisher-Price Recalls Simplicity’s Rainforest™ Portable Play Yards Due to Fall and Entrapment Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Rainforest™ Portable Play Yards manufactured by Simplicity Inc. and SFCA Inc.

Units: About 200,000

Manufacturer: Simplicity Inc. and SFCA Inc., of Reading, Pa.

(Note: Simplicity Inc. and SFCA Inc. have not responded to CPSC’s request to recall these products nor have they been responsive to consumer complaints recently received.)

Hazard: One or more rails can collapse unexpectedly, posing a fall or entrapment hazard to young children.

Incidents/Injuries: Due to Simplicity Inc. and SFCA Inc. being unresponsive to consumers via their Hotline and Web site, a significant number of complaints were recently reported. There have been at least 1,350 reports of one or more rails collapsing. There were numerous reports of children receiving bumps and bruises when the side rail collapsed. There were five reports of injuries which included a broken nose, a broken wrist, a mild concussion, a cut to the hand which required stitches, and a chipped tooth.

Description: The recalled play yards are portable and were sold with a bassinet, changing table, and mobile features. The products have a Rainforest™ patterned fabric, Rainforest themed artwork and bear the “Fisher-Price” logo. The Simplicity Inc. model numbers are 5310 RNF, 5310RNFC, and 5310RNFW. The model number is located on a sticker on one of the legs underneath the play yard.

Sold at: Retailers and specialty children’s stores nationwide from January 2007 through January 2009 for about $100.

Manufactured in: China

Remedy: Consumers should stop using the play yards immediately. Although it did not manufacture or sell the play yards, Fisher-Price has voluntarily agreed to assist consumers who own the products. Consumers who own one of the recalled play yards with the Fisher-Price logo should contact Fisher-Price to obtain assistance to replace their play yards.

Giant Bicycle, Inc. Recalls Bicycles; Fork Can Break Causing Rider To Fall

Name of Product: 2009 model year TCR Advanced SL and SL (ISP) Bicycles and Frames

Units: About 1,000

Importer: Giant Bicycle, of Newbury Park, Calif.

Hazard: The density of the steerer tubes can cause the forks to crack and break, posing a fall hazard to the consumer.

Incidents/Injuries: Giant Bicycle has received one report of the fork cracking with no reported injuries.

Description: This recall involves 2009 TCR Advanced SL Team, SL 0, SL 1, SL 2, and SL (ISP) model bicycles and frames in silver, charcoal, blue and red. The words “Giant” and “TCR Advanced SL” are printed on the frame. Steerer tubes with “B”, “N” or “P” at the end of the serial number are not included in this recall. Other “TCR” model bicycles are not included in the recall.

Sold by: Authorized Giant Bicycle dealers nationwide from August 2008 through December 2008 for between $3,300 and $7,500.

Manufactured in: Taiwan

Remedy: Consumers should stop riding these bicycles immediately and contact an authorized Giant Bicycle dealer for a free inspection and replacement fork.

Cheyenne Industries Recalls Vanity Stools Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Milan Vanity Stools

Units: About 89,000

Distributor: Cheyenne Industries Inc., of Little Rock, Ark.

Hazard: The recalled stools can become unstable due to loose screws, which can cause the stool legs to separate or break, posing a fall hazard to consumers.

Incidents/Injuries: Cheyenne has received nine reports of incidents, including seven resulting in minor injuries from the stool legs separating or breaking.

Description: This recall involves the Milan Vanity Stool Model GBS57A. The stools are rectangular benches sitting about 18 inches off the ground, with fabric-covered seat cushions and metal frames. UPC Code 0-50276-85472-2 is printed on the identification tag on the underside of the seat cushion. Only stools using four screws to hold the stool legs to a foot ring are impacted by this recall.
